Whether you call, email or stop in the office, the LRS staff will welcome you with a smile and a knowledgeable, prompt response. During this initial contact the LRS staff will ask required in-take questions to ensure we provide you with the best possible care. These questions will include, patients name, reason for physical therapy, including script or referral details, best contact method, and insurance information. Once these details are taken LRS will schedule an initial exam. During this exam you will meet with a licensed physical therapist who will discuss your immediate concerns. With your consent, we will design the best therapy regimen. Before you leave the office, we will schedule appointments with you that fit your schedule so you can complete your plan of care. You will also be provided with an individually tailored home exercise program.

LRS participates with several insurance providers. Most insurances will require a script or referral. However, some insurances provide provisions for ‘Direct Access’ which excludes a script or referral. Please ask our staff if this option is available to you.

LRS is an in-network provider to many insurances, as well as a participant to several HRA/HSA programs., Below are some of the participating providers:

  • Various Blue Cross Blue Shield plans including: , BCBS Complete, BCBS Medicare Plus Blue, BCBS out of State and Federal, HMO and PPO policies
  • Blue Care Network plans HMO and PPO
  • 44 North Supplement Plan

  • Medicaid, PPO and HMO plans
  • Medicare, including many Medicare supplement plans

  • Premiere Provider referral status with US Veterans

  • Workman Comp and Auto Claims – State Farm, Progressive, York Management, Hastings Mutual, to name a few

LRS also accepts PHP (also known as Physicians Health Plan) PPO and HMO plans with the provision they carry ‘Out of Network Benefits’.

As a courtesy LRS, will check your benefits as it pertains to physical therapy, however, it is ultimately the responsibility of the patient to know and understand what their individual plan(s) will cover.

LRS also provides ‘self-pay’ options. This allows for a patient experiencing chronic issues or who is in need of immediate pain management assistance to assume the cost directly and avoid insurance billing. Please all our office for further details.
